SSS and PhilHealth Voluntary Member Maternity Benefit Requirements

Here's the SSS and PhilHealth requirements for voluntary members, for claiming maternity benefit.

SSS Maternity Benefit Voluntary

* MAT-1:  Maternity notification duly stamped received from SSS prior to the date of childbirth.  Get this form on SSS, fill it up and submit to SSS.  Submit the form as early as possible.

* MAT-2:  Get this form from SSS.  Would be submitted to SSS after childbirth along with MAT-1 and other requirements.

* Must have at least three monthly contributions within the 12 month period immediately preceding the semester of childbirth.

* Birth certificate of the baby duly registered with the Local Civil Registrar (original and photocopy).

* SS ID, or if not available, bring 2 valid IDs.

* For cesarean delivery, you would need the Operating Room Record or Surgical Memorandum duly certified by the hospital where the member is confined (talk to your OB about this).


PhilHealth Maternity Benefit Voluntary

* CF1:  To be accomplish by the member.
* CF2:  To be accomplish by the doctor.
* PhilHealth Official Receipts or MI5:  Should cover 9 months contributions prior to the month of confinement.
* MDR: Member Data Record.  You can get this in PhilHealth.
